By making this reservation you acknowledge and agree to the following: *

I am aware of the existence of risk in participating in Practice Room usage and accept these risks as my own. I acknowledge that my actions have direct effects on my personal health and the health of the School of Music and Texas State University population.

I will not enter a room that I have not been given approval for, nor will I enter a room before the end of the allotted air exchange time.

I will not share my Practice Room with ANYONE, including a Collaborative Artist. For rehearsals with Collaborative Artists I will schedule a large practice space as available through the School of Music Front Office.

I will sanitize my designated practice area with the disinfectants provided BEFORE and AFTER I practice. While I am in common areas or while my practice room door is open, I will be fully masked per TXST COVID-19 Policies.

I will follow all policies and procedures as laid out by Texas State University and the School of Music.

I assume all responsibility and will not hold Texas State University or the School of Music responsible for any consequences of my actions.
